Understanding Fibromyalgia

Millions of Americans are plagued by pain, fatigue, and poor sleep, often times without any easy or straightforward explanation. They are not sure what is wrong with them and often begin to seriously doubt their sanity. Many have told us that they fell “like hypochondriacs,” while others are convinced they have contracted some rare, terrible disease that is escaping detecting by even the most sophisticated tests. This is certainly understandable since the patient’s symptoms are severe, but routine examinations and laboratory testing fail to find objective evidence for the patients’ subjective complaints. The patient with pain, fatigue, loss of motivation may be suffering from chronic soft tissue rheumatism problem known as fibromyaglia syndrome (FS). This is especially true is there are associated sleep problems, headaches, feelings of numbness and tingling, and tightness all over their body.

Just The Facts:

  • 80-95% of all Fibromyaglia occurs in females.

  • The most common complaint is widespread pain.

  • Common sites of pain or stiffness are low back, neck, shoulder region (including trapezius muscles), arms, hands, knees, hips, thighs, legs, and feet.

  • To be able to render the diagnosis of FS, pain must be present on palpation in 11 of 18 tender sites.

  • Fibromyaglia is often aggravated by cold or humid weather, anxiety or stress, overuse or over-activity, and poor sleep.

  • It is usually worse in the morning and evenings.

What Causes Fibromyaglia?

There are three primary types of fibromyaglia:

  1. Primary or idiopathic FS (PFS) – occurs in patients with no definable or demonstrable cause. They all seem to have sleep disorders in common.

  2. Secondary or concomitant FA (SCFS) – occurs in patients who suffer from another illness such as degenerative joint disease, arthritis, connective tissue disorders, chronic heart disease, lung disease, or hormonal imbalances.

  3. The third type of FS is termed post-traumatic fibromyaglia syndrome (PTFS) – occurs as a result of an accident or injury, often motor vehicle accident. PTFS patients are typically healthy, productive individuals until they are injured, after which time they develop musculoskeletal pain, problems sleeping, headaches, etc.

The clinician must be able to diagnose FS while ruling out other conditions as potential sources of pain. These conditions may include metabolic problems, inflammatory conditions (such as rheumatoid arthritis), infections, anatomic or biomechanical problems, neurological disorders, or other soft tissue problems. A common problem that may mimic FS, but is more localized, is myofascial pain syndrome. All the doctors at South Lake Medical Center have been trained to differentially diagnose all of these conditions.

Can It Be Treated?

Good treatment strategies are available at South Lake Medical Center. Patients must become active partners in their treatment, not merely passive recipients of care, in order for the treatment to be effective. In addition, patients must have support from family members and/or friends. We educate all our patients and family members in free health care classes. Patient and support member education is essential.

The combined multidisciplinary team approach to FS treatment has worked well for our patients. It is more effective getting the proper types of treatment in one setting than going to multiple health care providers in different locations at different times.
